Abstract
Age-related changes in the midface result from skin thinning, loss of volume in fat compartments, loosening of the facial ligaments, and bone resorption. To achieve satisfactory rejuvenation of the midface, the aesthetic practitioner needs to compensate for all of these factors. Presently, there is no single restoration technique that would address all aspects of aging. That is why a combined protocol should be implemented. Since 2019, all patients of the Clinic have been offered a combined protocol, or a step-up approach known as the “beauty-ladder”, as a means of improving the appearance of their midface.
The protocol commences with the use of biostimulators to improve the quality of the skin, followed by volume loss restoration and tissue elevation at the subsequent sessions. Improvement in overall appearance was evaluated according to the patient’s satisfaction, reported on a Likert-type scale. 75% of the patients were very satisfied with the outcome, and 25% were satisfied.
The “beauty-ladder” is a personalized protocol that allows to achieve complete rejuvenation of the middle face. A combination of different techniques and levels of restoration is necessary to achieve a satisfactory result.

Streszczenie
Proces starzenia się twarzy obejmuje zmiany we wszystkich strukturach anatomicznych.
Wraz z wiekiem dochodzi do utraty gęstości skóry, objętości w przedziałach tłuszczowych, rozluźnienia więzadeł twarzy i resorpcji kości. Aby osiągnąć zadowalające efekty odmłodzenia w środkowej części twarzy, lekarz musi brać pod uwagę wszystkie te czynniki. Obecnie nie ma jednej zalecanej metody, która pozwoliłaby zwalczać wszystkie aspekty starzenia. Dlatego też należy rozważać stosowanie protokołów łączonych.
Od 2019 r. wszystkim pacjentom kliniki oferuję protokół łączony (zwany „drabiną piękna”) jako najbardziej optymalny sposób na poprawę wyglądu. Protokół rozpoczyna się od stosowania biostymulatorów tkankowych w celu poprawy jakości skóry, a następnie na kolejnych sesjach następuje przywrócenie objętości i repozycjonowanie tkanek.
W opisanym badaniu poprawę ogólnego wyglądu oceniono na podstawie zadowolenia pacjenta przedstawionego w skali Likerta; 75% pacjentek było „bardzo zadowolonych” z wyniku, a 25% było „zadowolonych”.
Przedstawiony w artykule spersonalizowany protokół, polegający na połączeniu różnych technik i poziomów odbudowy, pozwala osiągnąć całkowite odmłodzenie środkowej części twarzy.
